 To our valued patient family,

1) You should have received a reminder for your appointment via text, email or phone to confirm your appointment.

2) We ask that you verify with your insurance company that we are in network with your plan and that you are eligible for your examination and materials prior to your visit with us.  This greatly assists with the check in process and streamlines your being seen promptly.

3) Masks are optional in the office during your visit.

4) If you are exhibiting any cold/flu/covid symptoms on the morning of your visit or have been exposed to someone with Covid 5 days prior to your visit we kindly ask that you reschedule your appointment.

Office hours have resumed to our normal schedule.
